The registry enables a community or region-wide visibility and access to a patient’s critical Advanced Care Planning (ACP) documents, including primary care physician practices, specialist physician practices (e.g. oncologists), acute care hospitals, skilled nursing facilities (SNFs) or nursing homes, home health providers, and, most importantly, emergency care providers (EMT).
Out-of-date or missing documents are the last things loved ones want to deal with in emergency or end-of-life situations. Providers get the right information at the right time to help make the right decisions.
Having access to the right documentation allows providers to honor patient's wishes, while reducing risks and avoiding potentially costly treatments.
